Program description

The Youth in Transition Worker (YITW) program supports youth ages 16 to 24 as they transition out of the child welfare system and into adulthood. The program is tailored for youth with a history in care who may also be navigating mental health challenges, substance use, housing instability, or justice involvement.

 

YIT Workers help youth develop and pursue their personal goals, while guiding them through adult systems and services. Support areas include housing, education, employment and training, health and mental health services, legal services, and life skills like financial literacy and household management. Services are youth-driven, short-term, and focused on building each youth’s strengths and independence.

 

YIT Workers meet youth where they’re at — whether in the community, at shelters, or virtually via text, phone, or email.

 

Participation is voluntary, flexible, and grounded in the youth’s own needs and goals. While the YITW role is supportive and goal-oriented, it is not case management in nature.
