1080 Queen St. East is a 9-unit Cluster Care site for seniors through a partnership between WoodGreen Community Services and Michael Garron Hospital. This innovative Cluster Care location provides an affordable and supportive living option for seniors who need assistance but do not require long-term care.
Eligibility
This WoodGreen Assisted Living location supports clients who are:
Seniors aged 59+ who need help with basic personal care, as well as more complex support involving planning or cognitive challenges
Individuals with a completed Medical/Functional Assessment form, signed by a medical professional
Clients who have also submitted applications through other hospitals or community agencies beyond WoodGreen
Support services offered in this location:
Personal Care Support (assistance with bathing, dressing, and other personal hygiene)
Home assistance (laundry, light housekeeping, grocery shopping)
Meal preparation
Medication assistance
24-Hour staff who can respond to emergencies
Social recreational programs

1070 Queen St E., Jack Layton Seniors Housing, offers 175 independent living units and three separate Cluster Housing areas for seniors who need closer support.

Cedarvale Ave. offers 59 units to seniors experiencing poverty and who have been precariously housed or unhoused.

Eligibility: Seniors 55+ experiencing or at risk of being unhoused, who are able to live independently and have applied through the City of Toronto Coordinated Access/PATHS waiting list.
Operated in partnership with Fife House, Wellesley Central Residences has 112 mixed units and offers supportive housing for Fife House clients living with HIV/AIDS and seniors receiving WoodGreen assisted living services. It also hosts an 11-unit transitional housing program for individuals living with HIV/AIDS and who are working toward permanent housing.
These programs offer individual, clinical supports to people aged 55 and older living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. 
Our team delivers hot and frozen meals to seniors and adults living with disabilities. We also provide at-home security and wellness checks and help with accessing local services.

On-call crisis intervention and outreach for older adults (65+) with dementia, addictions or mental illness. Our response team is available daily from 9 a.m. – 5 p.m.

We offer supervised group activities for dependent adults living with Alzheimer’s, disabilities or frailty. On-site support services include leisure activities, meals and personal care.
