Program description
WoodGreen’spersonal support services are available to seniors living in their own homes or in WoodGreen’s seniors-only residences.Our team of personal support workers provide personal care, meal preparation, medication reminders and household support.

Is there an age eligibility for the program?
Clients need to be 55+ to access the community programs
Clients need to be 59+ to access Assisted Living Services
What type of services can the PSW/HHW’s provide?
Care will be outlined in the care plan and will include activities of daily living (ADLs) and instrumental activities of daily living (IADLs). All clients within the programs should be able to self-direct their care needs.
What are examples of ADLs & IADLs?
Personal care, medication reminders/assistance, meal preparation, light housekeeping, laundry, support with toileting (clients should be able to do so with minimal assistance).
Are there any restrictions to the visits?
Client cannot smoke while staff are providing service. Clients must provide cleaning products and related equipment/supplies (mop, vacuum).
Can service be provided if the client is not at home during the scheduled time?
No, client must be in the home/unit for the services to be completed.
What happens if I go to hospital?
Services will be put on hold and will resume upon discharge home.
What if my needs change and I need more support?
WoodGreen will work collaboratively with Home and Community Care Support Services to address the needs.
How can I start receiving services?

Do I apply for the clusters through the housing department?
Cluster applications are received by the Client Selection Committee when new units become available and do not go through WoodGreen Housing.

2353 Dufferin St - Lewis Garnsworthy Residence, managed by WoodGreen Community Services, offers a mix of affordable market and Rent-Geared-to-Income units for seniors 59+.

1070 Queen St E., Jack Layton Seniors Housing, offers 175 independent living units and three separate Cluster Housing areas for seniors who need closer support.

Cedarvale Ave. offers 59 units to seniors experiencing poverty and who have been precariously housed or unhoused.

Eligibility: Seniors 55+ experiencing or at risk of being unhoused, who are able to live independently and have applied through the City of Toronto Coordinated Access/PATHS waiting list.
Operated in partnership with Fife House, Wellesley Central Residences has 112 mixed units and offers supportive housing for Fife House clients living with HIV/AIDS and seniors receiving WoodGreen assisted living services. It also hosts an 11-unit transitional housing program for individuals living with HIV/AIDS and who are working toward permanent housing.
These programs offer individual, clinical supports to people aged 55 and older living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. 
Our team delivers hot and frozen meals to seniors and adults living with disabilities. We also provide at-home security and wellness checks and help with accessing local services.

On-call crisis intervention and outreach for older adults (65+) with dementia, addictions or mental illness. Our response team is available daily from 9 a.m. – 5 p.m.

We offer supervised group activities for dependent adults living with Alzheimer’s, disabilities or frailty. On-site support services include leisure activities, meals and personal care.
