Building description
53 & 63 Pape Ave.offer a total of 20 units and a shared living space, which serves seniors in need of full-time support. WoodGreen’s Community Care Unit provides 24-hour assistance for daily living.
Eligibility
This WoodGreen Assisted Living location supports clients who are:
Seniors aged 55+ who need help with basic personal care, as well as more complex support that involves planning and cognitive skills.
Clients having a Medical/Functional Assessment form completed and signed by a medical professional.
Clients with completed applications with other hospitals and local community organizations other than WoodGreen.
Support services offered in this location:
Personal Care Support (assistance with bathing, dressing, other personal hygiene)
Home Assistance (laundry, light housekeeping, grocery shopping)
Meal Preparation
Medication Reminder/Assistance
24-Hour staff who can respond to emergencies
Social recreational programs

1070 Queen St E., Jack Layton Seniors Housing, offers 175 independent living units and three separate Cluster Housing areas for seniors who need closer support.

Cedarvale Ave. offers 59 units to seniors experiencing poverty and who have been precariously housed or unhoused.

Eligibility: Seniors 55+ experiencing or at risk of being unhoused, who are able to live independently and have applied through the City of Toronto Coordinated Access/PATHS waiting list.
Operated in partnership with Fife House, Wellesley Central Residences has 112 mixed units and offers supportive housing for Fife House clients living with HIV/AIDS and seniors receiving WoodGreen assisted living services. It also hosts an 11-unit transitional housing program for individuals living with HIV/AIDS and who are working toward permanent housing.
These programs offer individual, clinical supports to people aged 55 and older living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. 
Our team delivers hot and frozen meals to seniors and adults living with disabilities. We also provide at-home security and wellness checks and help with accessing local services.

On-call crisis intervention and outreach for older adults (65+) with dementia, addictions or mental illness. Our response team is available daily from 9 a.m. – 5 p.m.

We offer supervised group activities for dependent adults living with Alzheimer’s, disabilities or frailty. On-site support services include leisure activities, meals and personal care.
