The Comprehensive Care and Integration Specialist Team has evolved from HealthLinks and the (Transitional Care Coordination Virtual Hub). Our hope is that participants will find value in better understanding the journey through these various phases, and the partnership work that has gone into the program. Our hope is that this model of care can be explored and possibly replicated.
