The LSRT Case Manager Student will support the Living Standards Response Team (LSRT) in assisting tenants experiencing housing stability, safety, and living standards challenges. This placement provides hands-on experience in case management, supportive housing, harm reduction, eviction prevention, crisis response, and interdisciplinary service coordination while working alongside experienced professionals dedicated to promoting tenant well-being and housing stability.
Responsibilities
• Support intake, triage, and follow-up activities for tenants referred to the Living Standards Response Team.
• Assist with information gathering, client assessments, case planning, and service coordination.
• Shadow Case Managers during tenant visits, case conferences, and interdisciplinary meetings.
• Assist in developing and implementing tenant-centred action plans that promote safety, housing stability, and improved living conditions.
• Support referrals to internal and external community resources.
• Maintain accurate documentation, case notes, and administrative records in accordance with departmental procedures.
• Participate in team meetings, case discussions, and learning opportunities as assigned.
You Must Be
• Currently enrolled in a diploma or degree program in Social Service Work, Mental Health & Addictions, Community Services, Social Work, Psychology, or a related field.
• Demonstrated interest in case management, supportive housing, community-based services, and working with vulnerable populations.
• Ability to maintain professionalism, confidentiality, and appropriate boundaries in a client-centred environment.

Shift Hours
• In-person placement.
• Monday to Friday, 9:00 a.m. – 5:00 p.m., including one late day per week from 11:00 a.m. – 7:00 p.m.
Commitment
• Minimum of 14 hours per week.
• Three-month placement commitment, or as required by the student’s academic program.
