Provide crisis support and case management to seniors through phone, in-person, and home visits. This position is open to start in Fall 2025.
Responsibilities:
- Telephone and in-person assessments for seniors in crisis
- Crisis-based case management support for older adults and seniors
- Home visits in pairs to seniors living in the community
- Crisis counseling for seniors and their caregivers
You must be:
- Openness to visits in seniors’ homes where living conditions are not ideal
- Interest in working with complex seniors
- Ideally studying in the field of Social Work (BSW or MSW)
What You Bring to the Team:
- Strong communication and interpersonal skills
- Strong crisis intervention and counseling skills
- Collaborative approach to supporting outreach, evaluation, and engagement
- Knowledge of senior-specific issues and community resources
Shift hours:
- 21 hours a week (ideally the practicum will be 5 days a week, but 3+ days a week will work also be acceptable)
Location:
- Office at 815 Danforth Avenue, Toronto. Practicum includes visits to the community across the City of Toronto.
