Program description

Using Recovery Model principles, this program addresses the physical and mental health needs of homeless/under-housed people who use hostels, shelters and drop-in centres in downtown Toronto. Shared Care Clinic Outreach Services are offered at 650 Queen St. E., where individuals can meet with a CAMH nurse to connect with services and long-term supports for physical and mental health, housing, financial issues, recreation and vocational opportunities. Psychiatric assessments are available upon referral.

 

This program is available only by referral from WoodGreen staff.
