If you are living with a developmental disability and are interested
in accessing more support for yourself, we can help. WoodGreen's
Developmental Services are aimed at helping adults with a range of
developmental challenges to better integrate into the community and
to live more independently. We offer case management services,
including referrals to various community resources, social and
recreational groups, advocating on our clients' behalf and one-one-one
support with daily living to improve the quality of their lives.

Our services are aimed at supporting independent living for individuals
with developmental disabilities. We do this through promoting the
integration of our clients within the community, and helping them feel
less isolated and more empowered to make better decisions for themselves.
We can help you:

- Learn important life skills
- Get involved in more social and recreational activities
- Live independently, while being connected to your community

Our clients are adults that are at least 18 years of age who
have developmental challenges and are living primarily in East
Toronto. Self-referrals are encouraged, although referrals
from families or other services providers are also accepted.

Case Management Services

Through WoodGreen's Adult Protective Services case management program,
we advocate and help individuals living with developmental diabilities
to access the various community-based services they need including:
- Ontario Disability Support Program
- Affordable housing
- Medical services
- Social and recreational groups
- Financial and banking services
- Employment training and job opportunities
We also offer support to clients in helping to resolve personal issues, and to build important
life skills such as problem solving, budgeting and making friends.
Social and Recreational Activities

WoodGreen's Social Group meets one afternoon each week, providing individuals
who are living with a developmental diability with the opportunity to socialize
with others, develop new skills, and have fun in a comfortable environment.
Specifically we offer acivities such as:
- Bingo
- Arts and crafts
- Cooking
- Life skills workshops
- Group outings into the community
One-On-One Support with Daily Living

For individuals with developmental disabilities who require unique and
individualized help to live independently within the community, WoodGreen
offers the Supported Individualized Living (SIL) program. Providing up to
ten hours a week of one-on-one support to clients, WoodGreen staff offer
innovative and comprehensive case management services, support with daily
living and personal care assistance in client's homes or in other daily
living activities.

If you have a child who has been diagnosed with
one or more developmental delays, we can help.
WoodGreen's Parent Outreach Program provides
FREE in-home support to parents to help their
child gain independence, community living skills
and social acceptance. Our specially-trained
home visitors are all parents who have children
with developmental delays, so they understand
first-hand the needs and concerns of raising
a child with a developmental delay.

If you have a child, between the ages of 2 ½ and 21, who
has one or more developmental delays, such as autism,
PDD or global developmental delay and you:
- feel your child is having difficulty learning self-help
and social skills
- are concerned that your child is demonstrating
behavioral problems
- feel frustrated that you can't seem to get your child
to move forward
- need to talk to someone who really understands
the day-to-day challenges of raising a child with a
developmental delay

WoodGreen's home visitors will come to your home once
a week to help you learn new ways to support your child's
positive development, including:
- helping your child acquire life skills such as
daily routines, personal hygiene, dressing and
eating independently
- providing you with on-going support and
guidance in raising a child with a developmental delay
- helping to develop a closer relationship between
home and school
- providing you with information on other
services and resources available to you and your
children, including:
- respite care
- recreational programs and summer camps
- funding you may be eligible for
- speech and language therapy
- and much more

If you are interested in getting support from the Parent
Outreach Program or learning more about it, simply call
416-469-5211, ext. 1167. A Home Visitor will meet with you
weekly to identify your child's abilities and to teach you
how to build on those strengths, so you can help your child
become more independent.
