The Call for Proposals is currently open for all grassroots community groups serving vulnerable seniors within the Toronto Central LHIN. The Call is open until June 28th at 12:00 p.m.
The Seniors’ Grassroots Community Outreach to Vulnerable Seniors Funding Initiative supports community-based projects located within the Toronto Central LHIN boundaries (see map) that target outreach services towards high-risk seniors’ population groups. This program provides seed funding for projects that directly or indirectly address the preventable reasons why vulnerable seniors end up in hospital emergency departments. This program is specifically focused towards organizations that have no or little continual funding sources. Projects can include a combination of activities that address different aspects of prevention while ensuring the capacity of the organization to accomplish the work. This funding is unique in the way that WoodGreen will work intensively with groups to ensure the success of the project’s implementation and evaluation. All applicants should understand these guidelines and should attend an information session or consult with the Project Manager before submitting a grant application form. Applicants who wish to be considered for funding beginning mid-July, pending on approval from the Ministry of Health and Long Term Care, must submit their application by June 28th, 2010 at 12:00 p.m.

The format of the session will provide an overview of the grant (i.e. priorities, performance measurements and past projects) and give time to allow groups ask specific project questions.
