Seniors Grassroots Outreach to Vulnerable Seniors Funding Initiative


The Seniors’ Community Outreach to Vulnerable Seniors Funding Initiative supports community-based projects located within the Toronto Central LHIN boundaries that target outreach services towards high-risk seniors’ population groups. 
 

This program provides seed funding for projects that improve the health outcomes of high-risk seniors showing results in Emergency Department (ED) prevention and diversion and/or better disease management for the seniors served. Funded projects need to increase the access to health supports for isolated and frail seniors through culturally appropriate outreach and support in the community.   This program is specifically focused towards organizations that have no or little core funding sources for seniors’ projects and has high volunteer commitment. This funding is unique in the way that WoodGreen will work intensively with groups to ensure the success of the project’s implementation and evaluation.

All applicants should understand these guidelines and should attend an information session or consult with the project manager before submitting a grant application form.  Applicants who wish to be considered for funding, beginning mid-July must submit their application by the 4th of June 2012, 5:00pm.

Who can apply?

Any not-for-profit organization including:

·                    Community based groups

·                    Cultural associations and support groups

·                    Tenants associations

·                    Religious groups

·                    Any group that offers supports for seniors, including organizations that are not yet incorporated

 

To qualify for this funding, an organization must be grassroots/community-based in nature. The following guidelines provide a broad definition of what it means to be a grassroots organization. Exceptions may be made in certain circumstances, if necessary.
 

Budget

1)   The overall budget for the organization has to be less than $1,000,000. 

2)   The budget for seniors programs has to be less than $250,000.

Scope 

1)   Grassroots organizations/groups need to demonstrate a high commitment from the local community through the development of a strong base of volunteers committed to delivering programs to seniors in their own community.   

2)   Grassroots organizations/groups are encouraged to partner with local community based health service providers that act as mentors, partners and project sponsor to the grassroots organizations/groups.  

Staffing 

1)   The number of full-time staff should be less than 10.  

Other funding  

1)   Organizations that receive funding from the Toronto Central Local Health Integration Network (TC LHIN) are not eligible for funding.  

2)   Any activity funded by the Ministry of Health and Long-term Care (MoHLTC) and the City of Toronto’s Community Service Partnership (CSP) will be carefully reviewed to ensure that there is no duplication of services.

Information Sessions

Information Sessions will be held at 815 Danforth Avenue on:

  • May 4 (Friday)              1:00pm – 2:00pm
  • May 8 (Monday)           12:00pm – 1:00pm

  • May 14 (Monday)         10:00am – 11:00am
  • May 18 (Friday)            12:00pm – 1:00pm
  • May 23 (Wednesday)   5:00pm – 6:00pm
  • May 28 (Monday)         1:00pm – 2:00pm
  • May 31 (Thursday)       5:00pm – 6:00pm

Please confirm through email if you plan on attending a session.  The format of the session will provide an overview of the grant (i.e. priorities, performance measurements and past projects) and give time to allow groups ask specific project questions.  Several options have provided to groups to accommodate the varying schedules of those who need to attend.

Seniors’ Community Outreach to Vulnerable Seniors Funding Initiative: 2012/13 Application

 Community Impact 2010/11

The Outreach to Diverse & Vulnerable Seniors program specifically works with community groups who outreach to seniors experiencing challenges in managing their own health in their home.  It is our belief that community groups are the most effective in connecting with certain isolated and marginalized seniors when they share the same language, cultural context or community circumstances.  Groups funded within this program were able to empower their local community to support vulnerable seniors in methods that were well received and produced tangible results for seniors.  There are many positive impacts of this program that are not captured by this report, in some part due to the challenges of measuring the intangible aspects of community building, individual empowerment, and positive social ties that are the foundations of this work within the community.  The reason why this program demonstrates such positive results for vulnerable seniors, shown throughout this report, is because of the dedication of community leaders and volunteers to address the identified need of seniors in their community.  Click here to download the 2010/11 Final Report
